Labs are required ever 12 months. Lab work includes comprehensive metabolic panel (CMP), complete blood count (CBC), thyroid levels (TSH and T4), and lipid panel. Please make sure labs are fasting (no food or drink 8-10 hours before having your blood drawn).

Complete the initial intake paperwork via the patient portal at www.onpatient.com. You will need to receive a portal invite from our company to sign-up.

In the patient portal upload your photo, driver's license, and prescription insurance card.

Lab results should be uploaded to the message center and sent via pdf in the patient portal.
